Provide the information requested and read the “Utah Valley University Activity Participation Agreement” AND, if applicable, the “Utah Valley University Activity Participation Agreement For Minors.” Once you have completed the form (including the “Informed Consent for Minors” if applicable) you will be prompted again to click the blue “Click to Sign” button at the bottom of your screen. When prompted, please enter your email and then click the blue “Click to Sign” button to sign this document. Upon entering your email, your registration will be complete. A confirmation email with a copy of what you just signed will be sent to you.
