• Student/Visitor Accident - UVU Injury / Accident Report Form
  • Email the accident report to UVU Risk Management, [email protected].
• State/UVU Vehicle Accident 
  • Those involved in an accident must notify UVU Risk Management, [email protected] and UVU Fleet Services, [email protected] or 801-863-8507. 

• UVU Employee Accident - Complete the UVU Employee Injury Form found on the workplace safety & notices section of myUVU.  

  • After accessing the myUVU page click on > Employees tab > Human Resources > Workplace Safety & Notices > Workplace Injury > UVU Employee Injury Submission.
