Report an Incident

UVU Employee or Student/Visitor Incident Reporting

• This is a single report form to report INJURIES, SAFETY CONCERNS, AUTO ACCIDENTS, and INSURANCE CLAIMS against UVU. This report form is intended for safety and insurance matters, including WORKERS' COMPENSATION. 
  • For other report types, visit UVU's Report and Support website.
• Employees should use this form to report workplaces injuries or illnesses. Seek emergency medical care if needed, then complete this form within 24 hours of the time of the incident. When possible, employees should first report incidents first to their supervisor and/or People and Culture. Lastly, they should complete this form.
  • If an employee has not yet received medical care, please see "Employee Injury Procedures" on People and Culture's website.
• Students and visitors should use this form to report incidents such as general liability claims against UVU. 
  • In general, UVU's general liability insurance does NOT provide coverage to students in the classroom. However, all claims are determined on a case-by-case basis by Utah State Risk Management. Claims must be formally submitted to the responsible department (People and Culture, Risk Management, etc.) such as via this form, for coverage considerations.

This form will automatically send a copy to Risk Management, People and Culture, and Safety.

 

State/UVU Vehicle Accident 

• Automobile accidents must be reported via Utah State Risk Management's "Accident Report Form".
