Because of federal mandates, each school year has different requirements that must be satisfied. Please select the school year and form carefully from the list below. Forms completed for an incorrect year can NOT be accepted and may delay your receipt of financial aid.
Except for the FAFSA and UVU Link Data Form which all students complete, students will be informed through UV Link Financial Aid Status or directly from a Financial Aid Counselor if they need to complete any additional forms. Check your status frequently and submit requested forms timely to complete your financial aid file. These forms are made available here for your convenience.
