It is a federal requirement that an Exit Interview is completed for your Perkins Loan when you leave Utah Valley University or drop below 6 credits.  You must complete the Exit Interview, even if you plan to attend college elsewhere and intend to receive more federal loans.

Holds will remain on your transcript and graduation until this federal requirement is completed.

If you choose to complete the exit counseling online, please follow the directions below.

1. Visit http://MappingYourFuture.org (no "www").
2. Click on the "Complete Online Student Loan Counseling" link on the left.
3. Click on the "Perkins exit" link.
4. Complete the steps to select your school, read the content and answer questions, and complete the student form.
5. Print the confirmation page (it has your rights and responsibilities) or note the confirmation number for your records.

This process takes, on average, 25 minutes.

>Your driver's license number, if applicable
>Name, address, telephone number, and employer for your parent, if applicable
>Name, address, telephone number, and employer for your spouse, if applicable
>Names, addresses, telephone numbers, and employers for two references residing at different addresses
